| Welcome to the DIVE Cincinnati forms and
information page. Included you will find all the information
and forms you will need to complete before being accepted into
the program.
In order to properly register for the DIVE Cincinnati program,
each diver must register online by clicking the "registration"
button below and to the left.
In addition, each diver must download and completely fill
out and return Medial Release Form available in Adobe .pdf format.
Please note that the Medical Release Form MUST be notarized.
Once online registration has been complete and the Medical
Release Form has been completed and notarized, your DIVE Cincinnati
registration will be complete.
